PMCC is looking for a self-directed and organized individual to join our team as the Adult Day Supervisor. This position is in charge of the overall operations of program services, development of relationships with community partners, and supervision of PMCC employees. Someone with group and time management skills, supervisory experience, and knowledge/experience in a health-related field will be highly prepared for this position. This position reports to the Senior Director of the division.
Responsibilities/Compliance
- Oversee all programming services provided
- Collaborate with the clinical team on care management
- Manage staff which includes hiring, training, and performance review.
- Fiscal responsibility and administrative decision making
- Manage a high performing team poised for growth; supervise up to 10-20 people
- Meet regularly with supervisees to ensure timely feedback, answer questions, and identify trends or issues such as training needs
- Shadow/observe employees at least twice annually to measure competency and provide clear feedback about communication and work approaches
- Ensure compliance with all contractual requirements, licensure regulations, and Agency policies
- Manage productivity of team members and ensure minimum productivity standards are met
- Address any employee performance
- Provide departmental-specific training for new and current employees using approved training materials and approaches to ensure health, safety, and protection of the rights of persons supported
- Lead regular team meetings; cascade updates and other information as required
- Ensure staffing needs are met; interview prospective candidates and make employment offers to applicants
- Manage program engagement; build relationships with community partners including case managers, prospective employers, and other stakeholders to grow the division
- Be a positive role model for other employees
- Demonstrate excellent written and oral communication skills/abilities
- Monitor and review program expenses and revenues; investigate issues and offer solutions to resolve financial problems (ie, Service Agreements, MA closure, Spenddowns, rate issues)
- Review and approve expenses for all direct reports
- Participate in regular meetings with the finance department and ensure all program financial procedures are followed and maintained
- Drive clients if needed
- Perform other duties as required or assigned
- Ensure excellence and quality services; promptly follow up as needed on record audits
- Complete documentation of any billable services provided in the selected electronic health record/database system within 1 business day of providing services
- As a mandated reporter, respond and report all incidents that occur while providing services to protect the health and safety of and minimize risk of harm to the person(s) serve
Qualifications
- Bachelor’s Degree in Human Services, Social Work, Psychology, Sociology, or a related field
- If you hold a Social Work degree, we expect you to be currently licensed as a Social Worker as required by the Minnesota Board of Social Work
- Three (3) years of supervisory-level experience in a direct support services program and within the prospective or a closely related program required
- Must be highly organized and have essential computer skills
- Knowledge of various human service practices is necessary
- Position requires excellent time management skills
- Regular travel to provider sites and home visits is required
- Must have a valid Driver’s License and reliable transportation to be used as needed for work related functions
- Motor vehicle check (MVR) with a satisfactory driving record required
- Approved Background check is required

Salary/Pay: DOE
Schedule: M-F, flexible during PMCC operating hours (8:30AM-3:00PM)

Licensed Practical Nurses will be responsible for supporting seniors and adults with disabilities at our adult day care center. As an LPN, you will promote the well-being, health and safety of residents in a person-centered care environment.
From this position, you will build your organizational skills and patient monitoring skills. You will also develop your cross-communication skills by connecting with a broad range of individuals varying in age, ethnicity and culture. Someone who is a self-starter, motivated and is able to multi-task will be best prepared to succeed in this position.
Responsibilities include:
- Monitoring basic patient health, such as taking vital signs
- Taking patient histories and maintaining documentation
- Providing personal care, such as helping with bathing and toileting
- Assist with tests or procedures
- Help develop care plans for clients
- Administering medication and treatments
- Give immunizations and other injections
- Attend, complete, and maintain current mandatory training requirements
- Attend, complete, and maintain additional training in medication administration, skill development, and positive supports
- Other duties as assigned
Minimum Qualifications:

- Licensed in Minnesota as a Practical Nurse with no restrictions

The position is responsible for the overall programmatic implementation of the person-centered approach. Peace of Mind Care Center serves seniors age 65+ and adults age 18+ who have a verifiable disability.
This position has duties that include, but is not limited to:
- Assist and/or complete intakes with person-centered served plan with team members.
- Developing and/or writing and implementation of each client’s person-centered support plans, (CSSP) Coordinated Service and Support Plan, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan.
- Maintaining all signatures as required by 245-D licensing rules and regulations.
- Maintaining, updating all files as required by 245-D licensing rules and regulations.
- Compliance with 245-D licensing rules and regulations.
- Writing or quarterly and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan.
- Able to problem solve as needed and/or be creative to problem solve.
- Coordination and ensuring the protection from abuse and general well-being of clients.
- Staff orientation, training, and supervision regarded to person-centered needs and services as written in their specific plan.
- The designated coordinator monitors target symptoms, attends psychiatric appointments, and communicates with all team members.
- Be available to receive calls during and outside of work hours from staff with questions or during emergencies.
Requirements:
- Ability to pass a background study conducted by the Department of Human Services (DHS);
- Qualify as a Designated Coordinator under 245D licensing rules;
- Valid driver’s license;

- At least 2 years of experience working with individuals who have intellectual disabilities and/or serious and/or persistent mental health concerns.
